Exosomes Are the Biggest Skin Care Trend of the Year—but Are They Worth the Hype?

Is it just me, or is skin care getting way more complicated? I love geeking out over the science, but lately it feels like every new trend sounds straight out of a sci-fi movie — from zombie cells to vampire facials. So when exosomes started popping up everywhere, from TikTok to dermatologist offices to beauty marketing, I had to know: Is this the next breakthrough ingredient I should add to my routine ASAP, or just another buzzy trend I can skip? I called my go-to dermatologist to separate the science from the hype.

What Are Exosomes?

If you've spent more than five minutes on BeautyTok lately, you've probably heard someone raving about exosomes. They're popping up in luxury serums, post-procedure treatments, dermatologist offices, and seemingly every beauty trend forecast for the year. But despite all the buzz, most people still have one question: What are they?

"Think of exosomes as tiny FedEx packages that cells naturally send to one another," says Dr Mona Gohara, a board-certified dermatologist. "Instead of delivering sneakers or birthday gifts, they deliver biological 'messages' made up of proteins, lipids, and genetic material that tell neighbouring cells what to do."

In skin care, those messages are thought to encourage the skin to repair itself, calm inflammation, and support collagen production. Unlike traditional ingredients that simply sit on the skin or stimulate it directly, exosomes are designed to support the skin's own communication and repair systems.

Why Is Everyone Talking About Exosomes Right Now?

Exosomes are riding the next wave of regenerative skin care — a category focused on working with your body's biology instead of simply adding another active ingredient to your routine.

"We've spent years talking about ingredients that directly stimulate the skin, like retinoids or vitamin C," says Dr Gohara. "Exosomes have generated so much excitement because they're rooted in our own biology. Rather than simply applying another ingredient, the idea is to support the skin's natural repair and communication systems."

That's what makes exosomes feel so different from many of the ingredients we've obsessed over in recent years. Instead of asking, "What can we add to the skin?" exosomes ask a different question: "How can we help skin do what it already knows how to do — just better?"

Dr Gohara compares it to improving communication on a construction site. Traditional skin care is like stocking a toolbox with better tools. Exosomes improve the communication between the people using those tools. It's a subtle shift in thinking — and one reason researchers are so intrigued by the technology.

That said, Dr Gohara is careful to point out that the science is still catching up to the excitement, especially when it comes to over-the-counter creams and serums.

What Are the Benefits of Exosomes?

Research suggests exosomes may help support the skin's natural repair process, calm inflammation, encourage collagen production, and speed recovery after cosmetic procedures like lasers and microneedling.

That's why you'll often hear about them in dermatologist offices. Many physicians use exosome-based treatments after procedures because they're designed to support the skin while it heals.

The strongest evidence so far comes from physician-administered treatments rather than products you can buy off the shelf. While over-the-counter exosome creams and serums are becoming increasingly common, researchers are still learning how effective they are compared with in-office applications.

Who Should Try Exosomes?

According to Dr Gohara, exosomes may be worth considering if you're looking to support skin repair, address visible signs of ageing, or help your skin recover after an in-office procedure.

They're not necessarily a must-have for everyone, though. If your current routine is working well, there's no reason to feel like you're suddenly missing the one ingredient that will transform your skin. Just because new products are launching left and right doesn't mean you need all of them.

Do You Still Need Retinol and Vitamin C?

Yes — and this is where Dr Gohara says it's important to keep expectations realistic. "Exosomes are an exciting addition, but sunscreen, moisturiser, retinoids, and vitamin C should still be the foundation of your routine," she says.

In other words, don't think of exosomes as a replacement for the ingredients with decades of research behind them. Instead, think of them as a supporting player that complements your existing routine.

If you're already wearing sunscreen daily and using proven ingredients like retinoids and vitamin C, you've already built a strong foundation. Exosomes may eventually become another helpful tool — but they're not the cornerstone of healthy skin.

What Should You Know Before Buying an Exosome Product?

With more exosome serums launching every month, it's easy to assume they're all backed by the same level of evidence. They're not.

Dr Gohara recommends approaching these products with curiosity — but also realistic expectations. While the early research is exciting, especially in clinical settings, scientists are still learning how well over-the-counter formulas perform in real-world use.

Who Should Skip Exosomes?

Exosomes aren't right for everyone. Dr Gohara recommends waiting until your skin has healed if you have an active skin infection. Anyone with active cancer or a history of cancer should discuss exosome treatments with their physician before trying them, and there isn't enough safety data to recommend them during pregnancy or while breastfeeding.

The Bottom Line

Exosomes are one of the most exciting developments in regenerative skin care because they're designed to support the skin's own repair and communication systems. The science is especially promising for helping skin recover after in-office procedures, but research on over-the-counter products is still evolving.

For now, the smartest approach is to think of exosomes as a promising addition — not a replacement — for the skin care basics we already know work. Keep wearing sunscreen, keep using your moisturiser and proven actives, and if you're curious about exosomes, consider them the newest member of the team rather than the star player. Your skin will thank you for a thoughtful, balanced approach.
